ALL *NEW* PATIENTS or PATIENTS WITH NEW INFORMATION UPDATES, PLEASE COMPLETE A  Health History form.

All form submission MUST be completed 3 BUSINESS days prior to scheduled appointment date. 

Patients that fail to submit forms prior to your appointment will be rescheduled.  

Our tooth brushing area is CLOSED. All patients MUST brush at home.

FOR PARENTS AND/OR GUARDIANS:
All patients are limited to only ONE parent/guardian when entering the office.

 ONLY PATIENTS WILL BE PERMITTED IN TREATMENT AREAS.
